New Student Registration Procedure

ANNOUNCEMENT Number: 238/F.15/MAIN/VIII/2009 REGISTRATION PROCEDURES FOR NEW STUDENTS FOR THE JUNE 2009 PERIOD OF THE 2009/ 2010 Complete payment of tuition fees at Bank NISP Universitas Widyatama by bringing proof of payment issued by the PUPd department and USM card, at least 50% of the total fees to be paid no later than August 18, 2009. Re-register at the Academic Department:

Bring the following files/documents:

a. 1 (one) folder.

b. USM card (shown to the officer).

c. Proof of payment/deposit received from Bank NISP.

d. 1 (one) recent 2 x 3 cm passport photo (write your registration number, name, and major on the back)

e. 4 (four) recent 3 x 4 cm passport photos (to be attached to Form-03-04)

f. Health Certificate.

g. Certificate of Color Blindness (for the Faculty of Visual Communication Design)

h. Certificate of Good Conduct from the Police (for those who graduated in 2008 and earlier) or school (for those who graduated in 2009).

i. 1 (one) photocopy of Birth Certificate.

j. 1 (one) photocopy of Diploma/STTB and National Examination Score List (DNUN) that has been legalized.

k. 1 (one) photocopy of Identity Card (KTP).

l. 1 (one) photocopy of a Name Change Letter (if the name on the birth certificate is not the same as the name listed on the STTB).

m. Specifically for prospective students who are accepted into the International Program class, attach 1 (one) photocopy of a certificate/statement of results/TOEFL score of at least 450.

Fill out the Re-registration Form (FRM03-04) and the Widyatama University Student Pledge Statement (UTAMA), (FRM03-05) stamped with a Rp. 6000 duty stamp and signed by the prospective student and parent/guardian. Fill out the NISP Bank Savings Account Application Form in the name of the prospective student and the Account Debit Authorization Letter in the name of the prospective student, signed on a Rp. 6,000 stamp. Submit all re-registration files/documents and NISP Bank account opening files to the Academic Staff. The Academic Staff will check the completeness of the re-registration documents/files. Note:

  • Documents that have been submitted cannot be retrieved.
  • Prospective students who do not submit their files by August 18, 2009 (for item J, no later than August 10, 2009) will be considered to have withdrawn.
  • Passport photo without school identification (OSIS logo and/or school name) with a collared shirt/T-shirt

4. Payment for the next semester (semester II) tuition fees will be made by debiting the student’s NISP Bank savings account. Payment for the first semester (odd) tuition fees for the 2009/2010 academic year must be made by October 6, 2009, at the latest.

5. Payments that have been made cannot be withdrawn for any reason except if accepted at a state university through the SPMB Reguker pathway (undergraduate level) subsidized by the government (not through the Extension, Parallel, Interest & Talent Search, Special Pathway, and similar pathways). after deducting 20% of the financial obligations for the first semester (voluntary contributions are not subject to deductions) and if the student does not pass the UAN, the tuition fees and voluntary contributions will be refunded in full (as evidenced by a Letter of Statement from the School), with procedures in accordance with the 2009/2010 Academic Year New Student Registration Information book, page 58).

6. It is recommended that you re-register at least 3 days before the re-registration deadline to avoid long queues.
